Financial Policy We will provide exceptional care in exchange for payment at the time of service. We do not accept insurance assignment or Medicare. We encourage you to contact your insurance carrier to determine that you have out-of-network benefits. We accept cash, check, Discover, MasterCard, and Visa. We can also assist with financial repayment plans.
Insurance and Medicare Information Your insurance policy is a contract between you and your insurance company. In order to provide the most appropriate and exceptional care we are not contracted with any insurance carriers. As of January 1, 2009, the Ankle & Foot Center no longer accepts Medicare, and Dr. Segler has elected to opt-out of the Medicare program. We believe that opting-out of insurance plans allows us to provide more time with patient education, offer better treatments, with less patient paperwork and a higher standard of service. Please call your insurance company to determine your out of network coverage and what process will be required for you to obtain reimbursement. Full payment will be due at the time of service and you will be responsible for all charges. We accept payments of cash, check, Discover, Mastercard and Visa. We will not bill your insurance company for service, but will provide a superbill for you to submit to your insurance company for reimbursement.
Educational Resources We firmly believe in the importance of patient education. We provide our patients with a variety of individualized discussions, educational handouts and access to information on our website. These resources will provide you with the understanding of your condition that will allow you to participate in your own recovery.
48 Reschedule Policy We understand that your time is valuable. We will make every effort to get you into our office as quickly as possible. Please respect our 48hour Reschedule Policy. You must call to reschedule at least 48 hours prior to your appointment. Canceled appointments will be assessed a $45.00 no-show fee. Repeated canceled appointments will result in discharge of the patient from our practice.
Minors All minors (those under 18 years of age) must be accompanied by a parent/legal guardian. This policy applies to all new patient evaluations, follow-up visit, and surgeries.
